
Diabetic Services
Rawlins County Health Center offers comprehensive diabetes education and support for people with diabetes (PWD) and their families. Our program is designed to help patients better understand diabetes while providing the knowledge, skills, and tools needed to confidently manage their health.
Education and support are focused on the essential areas of diabetes self-management, including healthy eating, physical activity, blood sugar monitoring, medication management, problem-solving, healthy coping, and reducing the risk of diabetes-related complications.
Services are provided by qualified RCHC healthcare professionals, including a Registered Nurse/Certified Diabetes Educator and a Certified Diabetes Educator, who work with patients to develop practical strategies for managing diabetes and improving overall health. Diabetes education services may be covered by Medicare, Blue Cross and Blue Shield of Kansas, and many other major health insurance plans. Patients are encouraged to contact their insurance provider to verify individual coverage and benefits.

Diabetes Education & Support Services
- Diabetes Education & Prevention — Education on the diabetes disease process, prediabetes, diabetes management, and strategies to help prevent or delay Type 2 diabetes.
- Nutrition Education — Guidance on carbohydrate counting, consistent carbohydrate intake, and healthy eating habits to support blood sugar management.
- Oral Medication Education — Information about how medications work, proper administration and storage, potential side effects, and safe medication use.
- Insulin Education & Management — Education on insulin types, storage, administration, dosing, sliding-scale use, and problem-solving.
- Blood Sugar Monitoring — Education on target blood sugar ranges, finger-stick testing, and continuous glucose monitoring (CGM), including assistance setting up compatible smartphone apps for clinic monitoring.
- Prevention of Long-Term Complications — Education and preventive strategies related to potential complications affecting the eyes, kidneys, nerves, teeth and gums, hearing, and cardiovascular health.
- Diabetic Foot Exams — Routine foot assessments to identify potential concerns and help prevent diabetes-related complications.
- Specialty Care Coordination — Referrals to and collaboration with specialists as needed, including endocrinology, cardiology, nephrology, and urology.
